(a) The applicant shall adhere to the following requirements concerning the license:

(1) The license shall be displayed so that it can readily be seen by visitors; and

(2) The license shall not be altered or transferred.

(b) The resident director shall submit a new application for a license when:

(1) The location of the physical plant changes;

(2) The department has revoked or denied the license and reinstatement is sought by the applicant;

(3) The license has lapsed; or

(4) Ownership of the experiential/wilderness facility changes.

(c) The resident director shall notify DCYF in writing, when a change in the name of the experiential/wilderness facility or a change of the resident director is proposed.

(d) The resident director shall provide DCYF with information about the basis for any proposed program changes.
