(a) An application shall be denied or provider certification revoked if:
(1) DCYF determines that the state does not
have a need for the service;
(2)
The applicant or provider, or the individual acting on the applicant's or
provider's behalf, submits materially false information to DCYF;
(3) The provider knowingly retained an
employee for whom there has been a conviction for a felony or any crime against
a person;
(4) The provider has been
convicted of a felony or any crime against a person, which has not been
annulled or overturned;
(5) There
has been disciplinary action taken against a provider by a licensing body or
professional society, a finding of civil liability made for professional
misconduct, or a finding of an ethical violation made by a state or national
professional association or any other state's regulatory board, which has not
been annulled or overturned;
(6)
There has been a cancellation of insurance by the provider's insurance
company;
(7) There has been abusive
or neglectful treatment of a child by a provider as determined by any state
statute;
(8) The foster care
provider has had their foster care license revoked;
(9) A local education agency has revoked
authorization for an educational transportation service provider;
(10) The provider fails to submit a copy of
renewed driver's license at the time of expiration; or
(11) The provider fails to comply with He-C
6343.
