Part He-C 1503 - HEALTH CARE FACILITY DISCHARGE DATA SET SUBMISSION REQUIREMENTS
Section He-C 1503.03 - General Requirements for Data Set Submission

(a) Health care facilities shall submit data to the department, or its agent, as standard claims transactions in a format compliant with the Official UB-04 Data Specifications Manual published by the National Uniform Billing Committee.

(b) Unless otherwise specified in He-C 1503.04, the Official UB-04 Data Specifications Manual shall be the code source to be utilized for discharge data submission.

(c) Unless otherwise specified in He-C 1503.04, data elements shall be required as defined by the UB-04 reporting standard in the Official UB-04 Data Specifications Manual.

(d) Data submissions shall be made using the ANSI ASC X12N 837 electronic file format pursuant to 45 CFR 162.

(e) Data submissions shall be made to the department or its agent utilizing secure socket layer (SSL) protocol. E-mail attachments and paper submissions shall not be acceptable.
