(a) For each client, all providers shall maintain supporting documentation for all services rendered, including the date and time of service, the duration of the service, and a progress note on each service rendered.
(b) The information in (a) above shall be entered into the VMS no later than 3 days following the delivery of the service.
(c) All providers shall maintain a client record, to include a treatment or recovery plan as applicable, progress notes, consent forms, and details of service provision.
(d) All providers shall obtain client consent prior to any service provision, including any electronic voucher referral procedure.
