(a) No services shall be provided which exceed those authorized by the voucher management system unless requested by the client's care coordinator and approved by the bureau.
(b) Bureau approval of services which exceed service limits shall be based on:
(1) The
clinical appropriateness or need for the requested services; and
(2) The availability of funds.
