(a) Applicants for provision of ATR services shall submit a completed application, "NH Access to Recovery Service Provider Application" (August 2011), along with required supporting documentation to the department.

(b) Applicants shall meet the applicable licensing, certification, and clinical standards for the level of care and services for which they are applying.

(c) Approval shall be for a period of 5 years, or until ATR funding is no longer available or the current ATR grant expires.

(d) Upon expiration of the current approval, applicants shall reapply for provision of ATR services in accordance with (a) above.
