Section He-A 404.01 - Care Coordinator Requirements

(a) Care coordinators shall be designated employees of the NH department of community corrections, a NH impaired driver program, the NH National Guard PTO Program, the DCSP, those ATR providers listed in He-A 406.01(a) (2) -(4) , or those agencies currently approved or under contract or agreement with the department to provide case, client, or care management.
(b) Care coordinators shall successfully complete training provided by the department in the GPRA survey, the voucher management system (VMS), brief screening tools and techniques, and service planning.
