(a) No client shall receive services funded through the NH ATR program without being referred from one of the following entities:
(b) Entities shall conduct a brief screening of potential clients in order to determine the need for ATR services using a formal screening tool designed to elicit responses from clients relative to their involvement with alcohol and other drugs.
(c) A clinical assessment to determine the level of care needed shall be required for each eligible individual, as follows:
(d) Individuals referred to the NH ATR program shall be assigned a care coordinator by the referring entity.
