(a) No client shall
receive services funded through the NH ATR program without being referred from
one of the following entities:
(1) One of the
following military programs:
a. The NH
National Guard Prevention and Treatment Outreach (PTO) Program; or
b. The Deployment Cycle Support Program
(DCSP);
(2) A NH
impaired driver program;
(3) The NH
DOC's division of community corrections (DOCC);
(4) Those ATR providers listed in
He-A
406.01(a) ; or
(5) The bureau.
(b) Entities shall conduct a brief screening
of potential clients in order to determine the need for ATR services using a
formal screening tool designed to elicit responses from clients relative to
their involvement with alcohol and other drugs.
(c) A clinical assessment to determine the
level of care needed shall be required for each eligible individual, as
follows:
(1) Clinical assessments shall be
conducted using one of the following tools:
a.
Addiction Severity Index (ASI); or
b. Global Assessment of Individual Needs
(GAIN);
(2) NH impaired
driver programs shall conduct clinical assessments on the individuals they
refer;
(3) DOCC shall conduct
clinical assessments on the individuals they refer or have an assessment
conducted by a person from an another agency who meets the requirements in (6)
below;
(4) The military programs in
(a) (1) above shall not conduct clinical assessments on the individuals they
refer and shall instead have an assessment conducted by a person from an
another agency who meets the requirements in (6) below;
(5) The referral entity shall be responsible
for obtaining the individual's consent to share assessment information with NH
ATR care coordinator;
(6) All
clinical assessments shall be conducted by one of the following:
a. A NH licensed alcohol and other drug
counselor (LADC);
b. A NH master
licensed alcohol and drug counselor (MLADC);
c. A NH licensed independent clinical social
worker that is also certified by the National Association of Social Workers
(NASW) as a certified clinical alcohol, tobacco, and other drugs social worker
(CCATODSW); or
d. An individual who
holds a certificate of proficiency from the American Psychological Association
(APA) in the treatment of alcohol and other psychoactive substance use
disorders;
(6)
Individuals conducting clinical assessments shall be trained on the proper use
of the assessment tool;
(7) A
clinical assessment shall be conducted within 2 weeks of the referral;
and
(8) A clinical assessment shall
include a level-of-care recommendation based on the American Society of
Addiction Medicine "ASAM Patient Placement Criteria for the treatment of
Substance-Related Disorders," Second Edition-Revised (May 5, 2007), for all
clinical services.
(d)
Individuals referred to the NH ATR program shall be assigned a care coordinator
by the referring entity.
