Section Hcp 401.05 - Application for Restoration of License

(a) Each audiologist whose license has been expired for not more than 2 years shall, pursuant to RSA 137-F:13, III, have their license restored upon submitting:
(1) A
completed renewal application as required by
Hcp 401.03;
(2) Payment of the $350.00
restoration fee; and
(3)
Documentation that the applicant has met the continuing education
requirements of Hcp 402.
(b) Restoration shall not be available to hearing aid dealers whose registrations have expired prior to renewal.
