Section Fis 808.10 - Regulated Shooting Area Permit Application

(a) Applicants for a regulated shooting area permit shall provide the following information on or with the application form:
(1) Name of
applicant;
(2) Applicant's street
address and mailing address;
(3)
Applicant's telephone number and email address, if applicable;
(4) Address of the preserve;
(5) A plan of the premises including:
a. Clear delineation of all boundary lines,
fences and roadways;
b. Location of
all buildings, physical improvements and bodies of water;
c. All areas determined to be shooting or
non-shooting areas; and
d. Total
acreage of parcel;
(6) A
copy of any lease and its terms, if applicable;
(7) A copy of the rules for the regulated
shooting area;
(8) Date of
application;
(9) Importation permit
number, if applicable;
(10) Federal
permit number, if applicable;
(11)
The name and address of municipality where notification of application was
sent; and
(12) Signature of
applicant.
(b) For renewal of an existing regulated shooting area, the permittee shall request a renewal of the permit in writing by including on or with the application form:
(1) The permittee's name and
address;
(2) A statement that there
have been no changes to the original application and that the operation is in
compliance with all of the fish and game regulations; and
(3) The annual report specified in
Fis
808.08.
(c) A permit fee in the amount of $150.00 shall be submitted with each application and renewal application.
