Section Fis 804.18 - Permit to Possess Wildlife Forms

(a) The applicant for a permit to possess wildlife shall provide the following:
(1) Name of applicant;
(2) Applicant's street address and mailing
address;
(3) Applicant's telephone
number and email address, if applicable;
(4) Address where the wildlife is to be
possessed;
(5) The purpose for
which the wildlife will be possessed;
(6) The wildlife species' common name, and
scientific name of each species which will be possessed;
(7) Date of application;
(8) Importation permit number, if
applicable;
(9) Federal permit
number, if applicable; and
(10)
Signature of applicant.
(b) Pemittees requesting renewal of a possession permit shall submit:
(1) A census
listing of the number and species of wildlife in possession by the permittee at
the time of renewal; and
(2) A
statement, signed subject to the penalties for making unsworn false statements
under RSA
641:3, that the permittee is in compliance
with the requirements of Fis 800.
(c) A permit fee in the amount of $25.00 shall be submitted with each application or renewal application.
