(a) No person, firm or corporation shall process uncooked, raw shell-on American lobster tails, also known as green lobster tails, whether frozen or unfrozen, that have been separated from the carapace, in this state without first obtaining a lobster tail processing permit from the executive director.

(b) Applicants shall provide the following:

(1) The applicant's:
a. Name;

b. Name of contact person(s) responsible, if firm or corporation;

c. Street and mailing address;

d. Date of birth;

e. Height and weight;

f. Hair and eye color; and

g. Day and night telephone numbers;

(2) The business, firm or corporation's:
a. Name;

b. Federal tax identification number;

c. Date of incorporation;

d. Street address;

e. Telephone number;

f. Type of business; and

g. Location within the facility where green lobster tails will be processed; and

(3) Applicant's signature subject to the penalties for unsworn false statements under RSA 641:3.
