(a) Any firm, corporation, or person engaged in the taking or landing of herring pursuant to Fis 603.07 or the processing of shrimp pursuant to Fis 607.01 shall obtain a permit from the executive director.
(b) The applicant shall provide:
(1) The business
owner's:
a. Name and maiden name, if
applicable;
b. Street and mailing
address, if different;
c. Height
and weight;
d. Hair and eye
color;
e. Home telephone number;
and
f. Signature of permittee
subject to the penalties for unsworn false statements under
RSA
641:3.
(2) The businesses:
a. Name of business;
b. Federal Tax Identification
number;
c. Date of
Incorporation;
d. Business street
address;
e. Business telephone
number;
f. Type of
business;
g. Name of contact
person;
h. Business name of
processing operations harvest sold to; and
i. Processing operations street
address.
(3) The
vessels:
a. Vessel owner, if different than
applicant;
b. Vessel owner street
address, if different than applicant;
c. Vessel name;
d. State of registration;
e. State registration or coast guard
number;
f. National Marine
Fisheries Service federal permit number;
g. Principal port;
h. Hull ID number;
i. Hull construction material;
j. Vessel length;
k. Year built;
l. Gross and net tons, if federally
documented vessel;
m. Horsepower,
if federally documented vessel;
n.
Hold capacity in tons, if federally documented vessel; and
o. Crew size.
