(a) Applicants for a resident or nonresident commercial shrimp license under RSA 211:49-e shall make application on a form provided by the department.

(b) The applicant shall include, in addition to the information required by RSA 214:8, the following on the application form:

(1) The applicant's:
a. Name and maiden name if applicable;

b. Street and mailing address, if different;

c. Height and weight;

d. Hair and eye color;

e. Date of birth; and

f. Home or personal cell phone number; and

(2) The vessel owner's name, home or personal cell phone telephone number and address if the applicant is not the owner;

(3) The following vessel information, if applicable:
a. Vessel name;

b. State of registration and number or coast guard number;

c. National Marine Fisheries Service federal permit number;

d. Principal port;

e. Hull ID number;

f. Hull construction material;

g. Vessel length;

h. Year built;

i. Gross and net tons, if federally documented vessel;

j. Horsepower, if federally documented vessel; and

k. Hold capacity in tons, if federally documented vessel;

(4) Crew size, if applicable;

(5) The type of gear; and

(6) The signature of applicant subject to the penalties for making unsworn false statements under RSA 641:3.

(c) A resident and nonresident commercial shrimp license shall be signed by the licensee.
