Section Fis 1102.08 - Moose Lottery Application

(a) Complete name and mailing address;

(b) Town and state of legal residence at the time of application;

(c) Current driver's license number or state-issued non-driver identification number and an indication as to whether this number has changed since the applicant last applied;

(d) Date of birth;

(e) Home telephone number and cell or work number;

(f) Moose hunting unit preferences;

(g) Whether he or she is willing to accept an antlerless-only permit; and

(h) Whether he or she wishes to apply for a bonus point only.
