(a) Any person wishing to possess moose or other protected wildlife under RSA 207:21, RSA 208:9, or RSA 208:14, shall obtain a NH special wildlife permit from a department conservation officer in order to possess wildlife.

(b) Applicants shall provide the following:

(1) The type and number of the license, if applicable;

(2) The species to be possessed or transported out of state;

(3) Where transported and the date of such transportation;

(4) The date and town where wildlife was killed;

(5) The sex, weight, and place of storage of wildlife;

(6) The name, address, and signature of the person who killed the wildlife, or in the case of a minor, the signature of the accompanying adult;

(7) Dated signature, signed subject to the penalty for making unsworn false statements pursuant to RSA 641:3;

(8) The cause of death; and

(9) Acknowledgment receipt of the wildlife species received relieving the New Hampshire fish and game department and its employees of any liabilities relative to the suitability for human consumption.
