(a) An applicant for a permanent disabled crossbow permit shall complete the top portion of the "Permanent Disabled Crossbow Permit" form, Rev. 01/23, available at disabled-crossbow-permit.pdf (nh.gov).

(b) The applicant shall have the bottom portion of the form completed by a physician or advanced practice registered nurse practitioner familiar with his or her medical condition.

(c) When both portions of the form have been fully completed, the applicant shall submit it in person or by mail to the fish and game department 11 Hazen Drive, Concord, NH 03301, together with a one-time administrative fee of $10.00.

(d) No holder of a permanent disabled crossbow permit shall:

(1) Use a conventional longbow, recurve bow, or compound bow while hunting in New Hampshire;

(2) Hunt in New Hampshire without the appropriate hunting or archery license for that season; or

(3) Be exempted from the application of any other provision of these rules relative to the hunting of game species in New Hampshire.
