Section Env-Wq 402.40 - Signature Required for Holding Tank Registration Applications

(a) The applicant and, if the applicant is not the owner of the facility and the property, the owner(s) of the facility and the property, shall sign the holding tank registration application.
(b) The signature(s) shall constitute certification by the signer(s) that:
(1) The information contained in or otherwise
submitted with the application is true, complete, and not misleading to the
best of the signer's knowledge and belief;
(2) The signer understands that:
a. The submission of false, incomplete, or
misleading information is grounds for:
1.
Denying the application;
2.
Revoking any application that is granted based on the information;
and
3. If the signer is acting as
or on behalf of a listed engineer as defined in
Env-C
502.10, debarring the listed engineer from the roster;
and
b. He or she is
subject to the penalties specified in New Hampshire law, currently
RSA
641:3, for making unsworn false
statements;
(3) The
signer agrees to comply with all applicable rules and all conditions of the
permit, if issued; and
(4) The
applicant agrees not to discharge until written permission from the department
has been received.
