(a) The groundwater discharge permit shall contain conditions for supplemental assessments, monitoring, record keeping, and reporting as required to assure conformance with these rules, including but not limited to the following:
(1) The
parameters to be monitored based on the regulated contaminants present in the
discharge or groundwater contamination area or used in the permitted activity;
and
(2) The frequency and location
of water quality monitoring based on the hydrogeologic characteristics of the
site, predicted rates of groundwater flow, and treatment methods
employed.
(b) As a condition the permit, the permittee shall agree to control the use of groundwater within the discharge zone so as to prohibit its use for drinking water.
(c) Prior to commencing operations at the facility, the permit holder shall submit the following to the department:
(1) An as-built site plan on 81/2
inches by 11 inches or 11 inches by 17 inches sheets;
(2) Boring logs and well construction details
of wells installed after permit issuance; and
(3) One complete set of water quality
results.
