Section Env-A 1810.06 - Signature and Certification Required for Applications

(a) The application shall be signed and dated by:
(1)
The applicant, if the applicant is an individual; or
(2) A responsible person, if the applicant is
an entity.
(b) The signature of the applicant shall constitute certification that:
(1) The applicant has read, and understands,
the New Hampshire asbestos management rules;
(2) The application has been prepared in
conformity with this chapter;
(3)
All information contained in the application, including any supplements
attached, is true, complete, and not misleading to the best of the knowledge
and belief of the signer; and
(4)
The applicant understands that the applicant is subject to the penalties in RSA
641, Falsification in Official Matters, if any material information is untrue,
incomplete, or misleading.
