(a) Petitions for a declaratory ruling on matters within the jurisdiction of the board shall be filed as a petition which meets the requirements of Den 207.01(b) and also contains the following information:
(1) The exact ruling
being requested;
(2) The statutory
and factual basis for the ruling, including any supporting affidavits or memor
anda of law;
(3) A statement as to
how and why the issuance of a ruling on this subject would benefit the
petitioner, other interested persons, and the public at large;
(4) The identity, including mailing addresses
when reasonably available, of specific persons whose interests would be
affected by the issuance or nonissuance of the ruling in question.
