(a) Allocations to institutions under the New Hampshire UAAP shall be made only to eligible institutions within the state which shall have executed an allocation agreement with the commission, provided a certificate of good standing and certificate of authority,and meet all the requirements of Csp 601.04.

(b) The commission shall make funds available to the eligible institutions based upon the distribution formula as described in Csp 602.01 and shall grant additional funds if the institutions comply with Csp 601.05(c).

(c) Participating institutions distributing awards in an amount exceeding the UAAP funds allocated in July shall apply for reimbursement and provide annually, by April 30, a report on the status of the UAAP.

(d) The report in (c) shall include the following information for each scholarship recipient:

(1) His or her name and state of residency;

(2) The period of attendance;

(3) His or her enrollment status, either full time or part-time, during the period of attendance;

(4) The amount of the award; and

(5) The expected family contribution.

(e) All participating institutions shall provide annually, by October 15 following the end of the fiscal year for which an allocation has been distributed, a final report on the status of the UAAP, including all the information listed in Csp 601.05(d).
