(a) To initiate an appeal, the aggrieved party from the hearing below shall file with the board a notice of appeal. The notice of appeal shall be signed by the aggrieved party or his or her lawyer or representative and shall be filed within 30 days of the date of the final decision or order by the licensing board.
(b) The notice of appeal shall contain the following:
(1) The name and address of the
appellant;
(2) The name and address
of the appellant's lawyer or representative, if applicable;
(3) If applicable, the name/s and address/es
of the complainant, and other parties if applicable, at the proceeding
below;
(4) The grounds for the
appeal, including:
1.A detailed explanation of
the appellant's objections to the decision; and
2.Description of errors in the
decision;
(5) Any
background facts that relate to the appeal; and
(6) A description of the relief
requested.
(c) The aggrieved party shall file with the notice of appeal a copy of the decision that is being appealed.
