(a) The only pleadings permitted shall be petitions and replies to petitions.
(b) A petitioner shall include the following on his or her petition:
(1) The name and
address of the petitioner;
(2) The
name and address of the petitioner's representative, if any;
(3) A concise statement of the facts which
warrant the relief requested from the board;
(4) The description of the action which the
petitioner wishes the board to take;
(5) A citation to any statutes, rules,
orders, or other authority which entitles the petitioner to the relief
requested; and
(6) The signature
and date required by Bcr
205.02(a).
