Section 706B.660 - Applications

1. A pleading requesting a privilege, right or authority from the Authority must be styled an "application."
2. An application must set forth:
(a) The full name, mailing address and
telephone number of the applicant and the full name, mailing address and
telephone number of the authorized representative or attorney of the applicant,
if applicable;
(b) All material
facts that the applicant is prepared to prove and upon which the Authority may
base a decision to grant the request;
(c) Required exhibits and such other exhibits
as the applicant deems appropriate;
(d) A request for the order, authorization,
permission, certificate, relief or permit desired; and
(e) A reference to the particular statutes or
regulations requiring or supporting the requested action.
