CERTIFICATES OF AUTHORITY
Section 695C.123 - Federal qualification: Information required
Current through December 12, 2024

Any organization which holds a certificate of authority issued by the Commissioner and is seeking qualification as a health maintenance organization pursuant to 42 U.S.C. § 300e - 9(c) or qualifies as a provider-sponsored organization by satisfying all the requirements set forth in 42 U.S.C. § 1395w - 25(d) and the federal regulations adopted pursuant thereto shall submit to the Division information concerning that qualification, including:
