Section 689A.330 - Permitted changes

An insurer may add to a form approved by the Division its own identification and information to facilitate the use of that form. An insurer, service corporation or prepayment organization may request amplification of information from a provider of health care when the information is necessary for proper administration and determination of benefit payments. If an insurer or prepayment organization needs a form which differs from its approved form, the differing form must be submitted to the Division for approval. The insurer or organization must state the reasons for the deviation on its request.
