Section 688C.050 - "Information capable of identifying the insured" defined

"Information capable of identifying the insured" means any information capable of identifying the insured, including, without limitation:
1. The address of the insured.
2. The telephone number of the insured.
3. The number of the facsimile machine at which the insured can be contacted.
4. The electronic mail address of the insured.
5. A photograph or other likeness of the insured.
6. The employer of the insured.
7. The employment status of the insured.
8. The social security number of the insured.
