FIDUCIARY ACCOUNTS
Section 683A.410 - Licensee to be named as trustee; signature cards

The licensee must be named as trustee on accounts maintained pursuant to NRS 683A.400. The signature card for the account must include language which has substantially the same effect as the following:

To: (Name of Bank)

The account referred to on this card and carried by the undersigned with you in the name of John Doe, Trustee, is an account maintained under the provisions of NRS 683A.400. In the event of death of the undersigned, the executor or administrator of his or her estate will be the successor trustee.
