Cancellation, Suspension or Revocation of Certificate
Section 645D.400 - Delivery of certificate to Division

If the certificate of a certified inspector is cancelled, suspended or revoked, he shall deliver his certificate to the Division. No refund will be given when a certificate is cancelled, suspended or revoked.
