If a licensee or registrant changes his or her legal name after his or her license or registration is issued, the licensee or registrant must submit to the Board, within 30 days after the change:

1. Proof satisfactory to the Board that his or her name was legally changed; and

2. Payment of the fee for a change of name on a license or registration set forth in section 45.
