Section 639.918 - Written record of refills; exception

Except as otherwise provided in subsection 3 of NAC 639.751, the pharmacist must maintain in chronological order a separate written record of each refill that includes:
1. The prescription number;
2. The date of each refill or authorization;
3. The number of dosage units; and
4. The initials of the pharmacist who fills the refill.
The written record must be maintained for a period of 2 years after the date of the last refill entered therein for a prescription.
