1. An anesthesiologist assistant shall make a note in the records of every patient for whom the anesthesiologist assistant assists in the practice of medicine concerning the services provided to the patient.
2. A note described in subsection 1 must include, without limitation:
(a) A clear indication of the times that the
anesthesiologist assistant provided care to the patient;
(b) The name of the supervising
anesthesiologist; and
(c) The date
of the anesthesia service.
