1. Before practicing as an anesthesiologist assistant, an anesthesiologist assistant shall provide to the Board:
(a) A physical, public address, which may be
a location of practice of the anesthesiologist assistant; and
(b) A mailing address at which the
anesthesiologist assistant prefers to receive correspondence from the
Board.
2. Within 30 days after any change to the information provided to the Board pursuant to subsection 1, an anesthesiologist assistant shall provide updated information to the Board.
