CERTIFIED APPLICATORS
Section 555.680 - Applications for certification of nonprimary principal commercial applicators and private applicators: Contents; proof of passing examination

1. An application for a certificate to apply or supervise the application of restricted-use pesticides must be made on a form supplied by the Director and must show the following:
(a) The applicant's legal name, home mailing
address, home street address, if different from home mailing address, and home
telephone number.
(b) The assumed
name under which the applicant does business, if applicable, business mailing
address, business street address and business telephone number.
(c) The applicant's employer, employer's
mailing address, employer's business address and employer's telephone
number.
(d) A statement of whether
the applicant is applying for a certificate as a certified non-private
applicator or a private applicator.
(e) A listing and description of certificates
held in other states.
(f) The
address where equipment and pesticides are stored.
(g) Any category in which the applicant
wishes to be certified.
2. The qualifications for certification are that the applicant must be 18 years of age or older and present proof of passing the examination.
