Section 483.485 - Voluntary surrender of license: Contents of form; license deemed surrendered under certain circumstances

1. A person who voluntarily surrenders his or her driver's license to the Department shall complete a form, approved by the Department, which contains:
(a) A statement that the license is being
voluntarily surrendered to the Department;
(b) A statement setting forth the reasons why
the license is being surrendered;
(c) The person's signature; and
(d) The date the license is being
surrendered.
2. A license shall be deemed voluntarily surrendered to the Department because of, but not limited to, the following reasons:
(a)
The license is held by a minor and the person who signed the application of the
minor for the license dies; or
(b)
The communication to the Department of confidential medical reasons that render
the driver either physically or mentally incapable of driving.
