Section 458.342 - Application for certification

An applicant for certification as a detoxification technician must submit to the Division a completed application. To be considered complete, an application must:
1. Be accompanied by a nonrefundable fee in the amount of $25; and
2. Include:
(a) Documentation verifying that the
applicant meets the qualifications set forth in NAC 458.341;
(b) A statement signed by the applicant
indicating whether the applicant has been convicted of a felony and, if so,
when and where the conviction occurred and a description of the
offense;
(c) Written verification
of the applicant's current employment, if any;
(d) Completed forms, approved by the
Division, verifying the applicant's employment experience; and
(e) Any other information required pursuant
to this chapter or chapter 458 of NRS.
