Section 453.470 - Information to be affixed to package for prescription

A person authorized by the Board to dispense controlled substances who fills a written prescription for a controlled substance listed in schedule II, III or IV, or an emergency oral prescription for a controlled substance listed in schedule II, shall affix to the package a label showing:

1. The name and address of the pharmacy;

2. The serial number and date of initial filling of the prescription;

3. The name of the patient;

4. The name of the prescribing practitioner; and

5. Any directions for use and cautionary statements contained in the prescription or required by law.
