Section 449A.630 - "Provider access application and agreement" defined

"Provider access application and agreement" means a document in the format prescribed by the Secretary of State which is filed with the Secretary of State by a provider of health care who wishes to become an authorized provider and which sets forth the rights and responsibilities of an authorized provider with respect to his or her use of the Registry.
