1. A person or governmental entity that operates a program may renew a certification issued pursuant to section 9 by applying to the Division in the form prescribed by the Division and paying the fee required by section 15. The application must be signed by the director of the program or his or her designee and must include, without limitation:

If the Division determines that an application for renewal submitted pursuant to this section is incomplete, the Division must notify the applicant of the additional information necessary to complete the application. If the applicant does not provide the additional information to the Division within 90 days after receiving such notice, the certificate shall be deemed expired.
3. The Division may inspect an applicant for the renewal of a certificate pursuant to section 37 to determine if the applicant is in compliance with sections 2 to 40, inclusive.
