1. A UCC search request must contain:
(a) The name of the debtor to be searched and whether the debtor is an individual or an organization. A search request will be processed using the name in the exact form it is submitted.
(b) The name and address of the person to whom the search report is to be sent.
(c) The appropriate search fee.
2. If a filer requests a search at the time a UCC document is filed, the search will be of the name of the debtor as set forth on the form used for filing the UCC document. The requesting party must be the remitter of the UCC document, and the search request shall be deemed to request a search that would retrieve all financing statements filed on or before the date the UCC document is filed.
