The rate schedules for each policy form of credit life or credit accident and health insurance shall be submitted with such forms in quadruplicate with a separate rate sheet for each form. In cases of combination filings (policy forms containing both life and accident and health) the rate schedules shall be filed separately. In addition each rate sheet shall state the form number of the policy with which it is to be used.
