Section 477-8-002 - EXCEPTION TO REQUIREMENT OF AN SSN

If an applicant/client refuses to obtain an SSN for a well-established religious objection, the Department may assign the individual a unique Medicaid identification number as an alternative.

002.01 Well-Established Religious Objection

An individual

1. Is a member of a recognized religious group or division of the group; and

2. Adheres to the tenets or teachings of the group or division of the group, and for that reason is conscientiously opposed to applying for or using a national identification number.
