Any individual, health care professional, or agency may refer a potential applicant to the Medically Handicapped Children's Program or Genetically Handicapped Persons Program.
001.01 INITIATING A REFERRAL. A referral for services is made by the applicant, any individual, health care professional, or agency. A referral is made by completion of the application or, at a minimum, providing the Department with the potential applicant's following information:
(1) Name;
(2) Date of birth;
(3) Parent or legal guardian's
name;
(4) Address;
(5) Phone number;
(6) Diagnosis or medical condition;
and
(7) Reason for
referral.

EMERGENCY REFERRAL. A hospital emergency department
who wishes to refer potential applicants must make the referral within five
days of admission to the hospital.
