Section 180-7-108 - RECORDS OF TELETHERAPY, REMOTE AFTERLOADER, AND GAMMA STEREOTACTIC RADIOSURGERY FULL CALIBRATIONS

A licensee must maintain a record of the teletherapy unit, remote afterloader unit, and gamma stereotactic radiosurgery unit full calibrations required by 180 NAC 7-073 through 7-075 for three years. The record must include:

(A) The date of the calibration;

(B) The manufacturer's name, model number, and serial number of the teletherapy, remote afterloader, and gamma stereotactic radiosurgery unit or units, the source or sources, and the instruments used to calibrate the unit or units;

(C) The results and an assessment of the full calibrations;

(D) The results of the autoradiograph required for LDR remote afterloader units; and

(E) The signature of the authorized medical physicist who performed the full calibration.
