Chapter 7 - MEDICAL USE OF RADIOACTIVE MATERIAL
Section 180-7-089 - RECORDS OF MISADMINISTRATION

A licensee must retain a record of misadministration reported according to 180 NAC 7-115 for three years. The record must contain:

(A) The licensee's name;

(B) Name of the individual involved;

(C) Identification number or if no other identification number is available, the social security number of the individual who is the subject of the misadministration;

(D) A brief description of the event;

(E) Why it occurred;

(F) The the effect, if any, on the individual;

(G) The actions, if any taken, or planned, to prevent recurrence; and

(H) Whether the licensee notified the individual, or the individual's responsible relative or guardian; and, if not, whether such failure to notify was based on guidance from the referring physician.
