To obtain a certificate that is not in addition to the license as an independent mental health practitioner or mental health practitioner, an applicant must submit a complete application provided by the Department and provide documentation demonstrating that the applicant meets the requirements of Neb. Rev. Stat. § 38-2128, 172 NAC 10, and this chapter. The applicant must submit:
(A) An official transcript directly from the educational institution or through an acceptable electronic transcript service, showing proof of the required degree; and
(B) Evidence of meeting the education requirements set out in 172 NAC 94-007.05.
