Chapter 15 - SIGNATORIES; PERMIT APPLICATIONS AND REPORTS
Section 122-15-004

Any person signing a document under Sections 001 or 002 of this Chapter shall make certification under penalty of law that he or she has personally examined and is familiar with the information submitted in the document and all attachments and that, based on inquiry of those individuals immediately responsible for obtaining the information, he or she believes that the information is true, accurate, and complete. Further, he or she shall certify awareness that there are significant penalties for submitting false information, including the possibility of fine and imprisonment.
