(1) Cardiac rehabilitation services are limited to the following cardiac events and diagnoses:
(a) myocardial infarction within
the preceding 12 months;
(b)
coronary artery bypass surgery;
(c)
heart-lung transplant;
(d) current
stable angina pectoris;
(e)
percutaneous transluminal coronary angioplasty (PTCA) or coronary
stenting;
(f) heart valve repair or
replacement; and
(g) chronic stable
heart failure.
