(1) A person wishing to receive a loan of specialized telecommunications equipment shall complete the application and return it to the address designated in the application form. Application may be made by the person wishing to receive equipment, by a parent or legal guardian of a person under the age of 18 years or by a person duly authorized by the person or by a court to act on the person's behalf.

(2) In the case of an unemancipated person under the age of 18 years, a parent or legal guardian must sign the application and must assume full responsibility for the equipment and services. This is not required in the case of an emancipated person under the age of 18.
